Consistently works cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers. Is responsible for the safety of residents under his/her supervision. Consistently assists with transporting residents to activity programs and other scheduled events. Identifies resident problems, concerns and/or changes in condition and reports them immediately to the charge nurse. Obtains information necessary to establish a relationship and care for the resident maintaining our philosophy of Person-Centered Care. Participates in the development of an individualized plan of care of assigned residents. Documents the care and treatment provided to the resident at time of service. Participates in activities for residents in their household, geared to the interest and abilities of their elders. What do I need for this role with Frederick Living ?
