Description

We are seeking a Nurse LPN or RN to oversee the health and wellness of our residents, in coordination with our Executive Director and House Managers. We care about your work-life balance and equip you with the tools you need to be successful at work.

Bachelor's degree NOT required

Knowledge & Qualifications:

• At least 2 years of experience in Long Term Care/Assisted Living
• Thorough understanding of DHS 83 and Nurse Practice Act
• Licensed LPN or RN in the State of Wisconsin
• Excellent leadership skills to maintain a motivated and stable team
• Excellent organizational and problem-solving skills
• Proficient communication skills

Responsibilities:

• Provides assessments for potential residents to determine their eligibility for placement
• Performs timely and accurate resident assessments, communicating changes to the Administrator for billing purposes
• Creates customized Care Plans for each resident, ensuring team members have been properly trained to execute all tasks associated with Care Plans
• Provides oversight of medication management to assure all MAR's are correct and that the residents receive medications timely and accurately.
• Ensures all charting is performed in a timely manner and in accordance with state regulations.
• Creates positive team member relations through staff appreciation, recognition of excellent performance, and retention through ongoing and open feedback with team members
• Ensures staff are fully trained, including nurse delegation
• Ensure compliance with DHS 83, in partnership with the Administrator.
