Join us to support our Community on our Assertive Community Treatment Program as part a NEW Team, PACT #2, at our Evergreen Club Facility as a Behavioral Health Clinician II. PACT is an intensive and highly integrated approach for community mental health service delivery. The multidisciplinary team shares a caseload of individuals who experience symptoms of the most serious forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness. The PACT Clinician II provides direct, supportive, and ongoing coordination of therapeutic services to assist individuals in achieving the highest level of stability and independence in their community. Services delivered are predominately provided in a setting of clients’ choice, recovery-focused and include but are not limited to case management, treatment planning, resource identification and assistance, identification and development of natural supports, advocacy, service monitoring and follow-up, collaboration with natural supports and other providers, and individual and group services. This position offers the option of additional compensation for on-call coverage outside of regular business hours and rotates weekend and holiday shifts. ESSENTIAL DUTIES & RESPONSIBILITIES includes but are not limited to the following: Provide trauma-informed, person-centered counseling utilizing evidence-based practices. Identify and include the client’s support network and applicable involved community partners in care coordination. Assist clients in identifying and accessing resources which will meet their needs. Assist clients in the development of healthy coping mechanisms. Provide client education on mental disorders and support with symptom management. Coordinate and collaborate with community resources on behalf of clients. Manage caseload and meet productivity standard in accordance with agency program guidelines. Develop and implement care plans, measurable objectives and obtainable goals, in collaboration with the client, evaluating clients progress and adjusting as needed. Conduct risk assessments, develop and implement safety plans and interventions as needed. Facilitate and co-facilitate client treatment groups. Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices. Operate as a member of multi-disciplinary team and ensure services are provided in the community and clinical setting in compliance with any treatment models for Clinician II positions in any specialty programs. QUALIFIED CANDIDATES WILL HAVE: REQUIRED: Master's degree in psychology, counseling, social work or related field from accredited college or university. Must apply for Agency Affiliation within 30 days of hire. PREFERRED: Two years of experience providing clinical behavioral health services preferred.
