Job Overview:

Frontier Healthcare Systems is seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our team on a contract basis. This role involves providing high-quality palliative care to patients with serious illnesses in various settings, including homes, hospitals, and other healthcare facilities within the Chicagoland area and its surroundings. The ideal candidate will have a strong commitment to patient-centered care and a deep understanding of palliative care principles.

Key Responsibilities:

  • Conduct comprehensive assessments of patients’ physical, psychological, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Provide symptom management and support for patients experiencing pain, nausea, fatigue, anxiety, and other distressing symptoms.
  • Educate patients and families about disease progression, treatment options, and palliative care principles.
  • Facilitate discussions around advance care planning and end-of-life decisions.
  • Coordinate with other healthcare providers to ensure seamless care transitions and continuity of care.
  • Document patient care activities accurately and timely in electronic health records.
  • Participate in team meetings, case conferences, and continuing education activities.

Qualifications:

  • Education: Master's or Doctoral degree in Nursing from an accredited institution.
  • Licensure: Current Nurse Practitioner (NP) license in the state of Illinois.
  • Certification: Board certification in a specialty area (e.g., Family, Adult-Gerontology) and certification in Palliative Care (preferred).
  • Experience: Minimum of 2 years of experience in palliative care, hospice, or related field.

  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency with electronic health records and telehealth platforms.
