Home Health - Full-Time RN Field Nurse

Hill Country Preferred Senior Care

Responsibilities:

  • Performs skilled nursing services under the supervision of a staff RN.
  • Observes, records, and reports to the supervisor on the general physical and mental conditions of the patient.
  • Administers prescribed medications and treatments as permitted by State regulations.
  • Assists the physician and /or staff RN in performing specialized procedures.
  • Assists the patient with activities of daily living and encourages appropriate self-care.
  • Prepares clinical and/or progress notes and incorporates them into the clinical record at least weekly.
  • Reports to the patient's physician and other appropriate personnel pertinent changes in the patient's condition.
  • Performs complex wound care if in-service is documented for specific procedure.
  • Performs routine vena-puncture to obtain labs only if documentation of competency by a staff RN is in the personnel record.
  • Receives and follows physician orders within realm of practice for an RN and within the standards of home health practice but may not alter the plan of care.
  • Participates in in-service program, attending at least two in-service educational meetings per year.
  • Participates in nursing report.
  • Participates in staff conference regarding patient care.
  • Participates in QAPI plan and process.
  • Supervises LVNs and HHAs
  • Makes the initial nursing evaluation visits
  • Continually evaluates the patient's nursing needs.
  • Initiates and coordinates the plan of treatment and necessary revision.
  • Instructs and supervises the patient's family in the care of the patient and maintenance of a healthy environment for the patient.
  • Provides those services requiring substantial specialized nursing skill.
  • Renders nursing care to homebound patients and perform treatments as ordered by the physician.
  • Assists the patient's family to recognize the daily living problems of the patient through all.

Qualifications:

  • Current licensure in the state of Texas or Compact license.
  • Preferably has one-year experience of nursing care in hospital or home health setting.
  • Current driver's license and vehicle insurance, and access to a dependable vehicle.
