We are seeking a highly skilled and experienced Home Health - RN Case Manager (PRN) to join our team. The ideal candidate will have a strong background in home health care and be able to provide exceptional care to our patients.

Responsibilities:

• Assess patient needs and develop individualized care plans
• Coordinate and manage patient care with other healthcare professionals
• Provide skilled nursing care to patients in their homes
• Educate patients and their families on proper care techniques and disease management
• Monitor patient progress and adjust care plans as needed
• Document patient care and progress in electronic medical records
• Communicate with physicians and other healthcare professionals regarding patient care
• Ensure compliance with all state and federal regulations

Requirements:

• Current RN license in the state of Texas
• Minimum of 2 years of experience in home health care
• Strong clinical skills and knowledge of disease management
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Experience with electronic medical records
• Valid driver's license and reliable transportation
