Responsibilities:

- Manage patient care for hospice clients, including assessing needs, developing care plans, and coordinating services.
- Utilize nursing expertise to provide quality patient care and ensure patient comfort and well-being.
- Collaborate with interdisciplinary team members to optimize patient outcomes and support families.
- Implement discharge planning strategies to facilitate smooth transitions for patients.
- Adhere to HIPAA regulations and medical terminology standards in all interactions.
- Utilize physiology knowledge to provide specialized care for patients in a hospice setting.
