We are looking for an addition to our amazing team! The Nurse Care Manager shall provide primary care consultation, and psychiatric care consultation, and work with the Behavioral Health Home, the Health Home Coordinator, and the member to provide other Section 92 services as necessary. Estimated hours 10 per week maximum.

This role will be providing light nursing consultation and care in the clients' homes (e.g., explaining procedures, stocking pill containers, educating on appointments, and the like). The population is primarily refugees/immigrants struggling with aging and/or chronic health conditions; interpretation is provided.

Responsibilities:

• Support and closely coordinate mental health care with the patient’s primary care provider and when appropriate, other mental health providers.
• Provide provider education about common mental health and substance abuse disorders and available treatment options consultant and communicate resulting treatment recommendations to the patient’s PCP.
• Participate in regularly scheduled (usually weekly) caseload consultations with the psychiatric
• Consultations will focus on a new clients to treatment or who are not improving as expected.

Qualifications:

• A Registered Nurse
• A Psychiatric Nurse licensed as a registered professional nurse by the state
• An Advanced Practice Psychiatrist
• A Mental Health Registered Nurse licensed as a nurse practitioner
• Or a Clinical Nurse Specialist by the state

This position reports to the: Clinical Director
