Job Description

• Provides direct psychiatric services to the ACT Team clients and adults in the Medical Services Program as needed at multiple District 19 clinics.
• Conducts comprehensive psychiatric assessments and evaluations to determine appropriate diagnosis and course of treatment.
• Makes referrals and recommendations to additional providers as appropriate.
• Provides or assures that all consumers receive appropriate medical & primary care screening, psychiatric evaluation and diagnosis, and that all medical / psychiatric care is appropriately & timely delivered.
• Submits prescriptions electronically, accurately and timely for patients.
• Follows all established agency policies and procedures.
• Completes comprehensive psychiatric evaluations for ACT program admissions.
• Contact with ACT program consumers at a minimum, on a quarterly basis.
• Participates in team meetings, treatment planning, recommendations to improve psychiatric engagement, and assessment of medical and psychiatric needs.
