Provides direct psychiatric services to the ACT Team clients and adults in the Medical Services Program as needed at multiple District 19 clinics.
Conducts comprehensive psychiatric assessments and evaluations to determine appropriate diagnosis and course of treatment.
Makes referrals and recommendations to additional providers as appropriate.
Provides or assures that all consumers receive appropriate medical & primary care screening, psychiatric evaluation and diagnosis, and that all medical / psychiatric care is appropriately & timely delivered.
Submits prescriptions electronically, accurately and timely for patients.
Follows all established agency policies and procedures.
Completes comprehensive psychiatric evaluations for ACT program admissions.
Contact with ACT program consumers at a minimum, on a quarterly basis.
Participates in team meetings, treatment planning, recommendations to improve psychiatric engagement, and assessment of medical and psychiatric needs.
