Job Overview

Join Horizon’s esteemed Inpatient Hospice facilities, including the Lawlis Family Hospice on the tranquil Columbia St. Marys Ozaukee Campus and Kathy’s Hospice nestled in the picturesque Froedtert West Bend campus. Our philosophy centers on honoring the human spirit while delivering compassionate and high-quality care to patients and their families. We are currently in search of a dedicated individual for a full-time, third-shift position (11:00 PM - 7:30 AM) who is eager to be trained and provide exemplary care within both facilities.

Key Responsibilities

• Deliver fundamental patient care and services as directed by the assigned unit, under the guidance of a registered nurse.
• Execute patient care tasks following established clinical procedures as instructed by the RN.
• Accurately collect and document patient data to assist in evaluations and ensure the integrity of medical records.
• Identify and communicate any abnormalities in patient responses to the RN.
• Recognize emergency situations and respond appropriately.
• Support patient education as delegated by the RN.
• Build a therapeutic rapport with patients through effective communication.

• Alertness to recognize and report deviations in patient conditions.

Qualifications

• High School Diploma or equivalent.
• Valid Wisconsin CNA Certification valid for Federally Certified Home Health Agencies and Hospices.
• Experience engaging with the public is mandatory.
• Current and valid Wisconsin Driver’s License along with a reliable and insured vehicle.
