We are currently in search of a dedicated individual for a full-time, third-shift position (11:00 PM - 7:30 AM) who is eager to be trained and provide exemplary care within both facilities.

Key Responsibilities
- Deliver fundamental patient care and services as directed by the assigned unit, under the guidance of a registered nurse.
- Execute patient care tasks following established clinical procedures as instructed by the RN.
- Accurately collect and document patient data to assist in evaluations and ensure the integrity of medical records.
- Identify and communicate any abnormalities in patient responses to the RN.
- Recognize emergency situations and respond appropriately.
- Support patient education as delegated by the RN.
- Build a therapeutic rapport with patients through effective communication.
- Strong interpersonal and communication abilities tailored for patient interaction.
- Capacity to follow directions and fulfill assigned patient care tasks efficiently.
- Alertness to recognize and report deviations in patient conditions.
- Proficiency in documenting patient information with accuracy and attention to detail.
- Quick and competent in emergency response scenarios.
- Commitment to reinforcing patient education as per RN delegation.
- High School Diploma or equivalent.
- Valid Wisconsin CNA Certification valid for Federally Certified Home Health Agencies and Hospices.
- Experience engaging with the public is mandatory.
- Current and valid Wisconsin Driver’s License along with a reliable and insured vehicle.
