- In-house therapy team, smaller daily caseloads with emphasis on optimizing time with patients, high acuity, AL and IL residents, and program and certification support!
- The Parks Senior Living provides short- and long-term nursing and rehabilitation services to the senior population in our community. The Parks Senior Living is fully committed to providing our residents and employees with a true family, home-like setting where all employees look forward to going to work to provide the highest quality health care services possible.
- Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
- Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
- Assist as needed with patient screenings for the need for Physical Therapy.
- Communicate progress and any barriers toward PT goals with interdisciplinary team members.
- Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
- Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
- Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.
- Current Texas licensure as a Physical Therapist required.
