About the Role
As a Client Care Coordinator, you will play a pivotal role in matching our clients with compatible caregivers who can provide the support they need to thrive at home. You will be the primary point of contact for our clients, ensuring their satisfaction and building trusting relationships.
Responsibilities
  • Conduct comprehensive client assessments to understand their needs, preferences, and lifestyle.
  • Develop personalized care plans in collaboration with clients and their families.
  • Match clients with compatible caregivers based on their needs, personalities, and schedules.
  • Schedule, monitor, and oversee caregiver services to ensure client satisfaction.
  • Maintain clear and consistent communication with clients, caregivers, and families.
  • Address client concerns promptly and professionally.
  • Conduct regular client satisfaction checks and update care plans as needed.
  • Maintain accurate and up-to-date client records in accordance with company policies.
  • Build relationships in the community with professional referral sources.
  • Uphold the highest standards of professionalism and confidentiality.
Qualifications
  • Minimum 1 year of experience in a customer service or care coordination role.
  • Strong interpersonal and communication skills, both written and verbal.
  • Excellent organizational and time management skills.
  • Proficient in Microsoft Office Suite.
  • Ability to build rapport and trust with clients from diverse backgrounds.
  • Passion for helping others and a commitment to quality service.
  • Valid driver's license and reliable transportation preferred.
