Job Overview :

We are seeking a compassionate and dedicated Personal Care Worker to join our growing team, providing care and support to our individual with mental health and individuals with disabilities in our Adult Family Homes.

The ideal candidate will be responsible for assisting with daily living activities and promoting independence and well-being.

Responsibilities :

• Develop and implement personalized care plans to meet the unique needs of each client.
• Provide assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.
• Offer support with medication management and patient monitoring.
• Engage in patient observation to identify any potential health issues or concerns.
• Deliver high-quality care, recognizing the unique challenges and needs of individuals with severe mental health.
• ensuring that clients receive the support and attention they need to thrive.
• Provide group home care, creating a welcoming and supportive environment for clients.
• Maintain accurate records and documentation of client care and progress.

Experience :

The successful candidate will have relevant experience in a healthcare setting, with a strong focus on care and support for seniors and individuals with disabilities.

Skills :

• Strong understanding of care plans and how to implement them effectively.
• Ability to lift and assist with transfers as needed.
• Experience working in home community care or similar settings.
• Proficiency in patient observation and monitoring.
• Compassionate and empathetic approach to dementia care.
• Proven experience in senior care, with a focus on promoting independence and well-being.
• Knowledge of social work principles and practices.
• Strong communication and documentation skills.

Job Types : Full-time, Part-time

Pay : $15.00 - $17.00 per hour

Expected hours : 40.00 per week

Schedule :

• 10 hour shift
• 12 hour shift
• 8 hour shift
• Day shift
• Evening shift
• Morning shift
• Night shift
• Rotating weekends

Experience :

Caregiving : 1 year (Required)

Shift availability :

• Day Shift (Preferred)

• Overnight Shift (Preferred)

Ability to Commute :

Green Bay, WI 54304 (Required)

Ability to Relocate :
