POSITION SUMMARY | ||||||||
This position is responsible for ensuring a comprehensive transition plan for each youth releasing from secure confinement is completed pre-release, and related post-release follow ups are conducted to ensure service connectivity. This position is responsible for collaborating with various internal and external stakeholders to build and maintain supportive community resources and services. | ||||||||
RESPONSIBILITIES AND DUTIES | ||||||||
·Attends 60- and 30-day pre-release YCRT meetings for all youth releasing within their assigned districts. ·Works in coordination with partners, community case managers, youth, and families to finalize the Transition Plan with detailed aftercare services. ·Provides post-release follow ups to all youth in their assigned districts beginning 2-weeks, 30 days, and 60 days to ensure connectivity to services and to help eliminate and/or reduce barriers the youth and family may experience. ·Responsible for maintaining and updating the Resource Database while cultivating new resources and partners within their assigned districts. ·Assists youth in job placement and career readiness, while continuing to seek out F.R.E.S.H start partners. ·Maintains appropriate and timely documentation to include YCRT participation, contacts with youth and families, subgroup meetings, project/program coordination, and other reports as required. ·Coordinates monthly Reentry Task Force subgroup meetings and supports the sub-group in organizing and carrying out the action steps developed in the meetings. Coordinate special projects and programs that the subgroup has identified. ·Provides coordination and support for the Reentry Task Force. ·Seeks new mentors for Georgia Youth Mentoring and provides training to those mentors. Acts as a liaison between the facilities and community staff with those mentors. ·Provides support to groups or programs initiated by ORS. ·Keeps the supervisor informed and works cooperatively with all staff and others as required. ·Works cooperatively with DJJ staff to visit community offices and provide training on the Reentry Process. ·Continues to grow professionally through outside conferences, training, and collaboration with professional colleagues. ·Performs other duties and assumes other responsibilities as assigned or required. | ||||||||
