We're on the lookout for a compassionate and skilled Care Coordinator to join our practice. As a Care Coordinator, you'll play a pivotal role in ensuring every patient receives top-notch care efficiently. Your expertise will be key in directing, planning, and supporting our practice to meet various care coordination and quality initiatives. What You'll DoGeneral Responsibilities:
Stay updated on payor and regulatory requirements, adeptly handling tasks even under pressure.
Participate in learning sessions and share valuable insights with fellow team members.
Collaborate with interdisciplinary teams to achieve our care coordination, quality, and financial performance goals.
Facilitate in-person and virtual meetings to communicate results and provide educational support.
Uphold confidentiality in patient, employee, and physician interactions, adhering to our ethical standards and mission.
Care Gaps ManagementMonitor patient compliance with preventive screenings using reporting tools.
Communicate effectively with patients regarding their care needs and maintain accurate records.
Assist in securing necessary documentation for patient assessments and identify potential behavioral health care gaps.
Transitions Of CareReach out to patients post-emergency department visits or hospital discharges to ensure follow-up care.
Document transition of care activities to meet regulatory requirements.
Care ManagementIdentify at-risk patients and refer them for necessary medical or behavioral health care management.
Assist in establishing comprehensive chronic care management for eligible patients.
Coordinate patient care plans in collaboration with Care Managers and facilitate referrals to specialists.
Population HealthAssist in practice and provider empanelment processes.
Schedule appointments for preventive care and chronic disease management.
Prepare and maintain care coordination reports and conduct wellness campaigns.
What You'll NeedGraduate of a certified healthcare education program preferred.
One year of relevant experience in medical offices or care coordination preferred.
Proficiency in Microsoft Office suite and electronic medical records.
Strong communication skills, both verbal and written.
Ability to prioritize tasks in a fast-paced environment.
Must be located in the Richmond, VA area.
