ResponsibilitiesManage a designated caseload under the guidance of Care Management Leadership.
Conduct regular outreach to enrollees based on their needs, ensuring timely and compassionate care coordination.
Perform in-person visits as required, meeting enrollees and caregivers in homes, medical offices, or other agreed locations.
Assess enrollees regularly to identify needs, address barriers, and ensure continuity of care.
Complete care coordination tasks efficiently and in compliance with organizational policies and industry standards.
Collaborate with Senior Care Managers to prioritize cases, set objectives, and mitigate potential risks.
Document interactions and findings accurately and promptly, respecting confidentiality guidelines.
Participate in discharge planning and transitions of care, ensuring seamless healthcare transitions for enrollees.
Educate enrollees and caregivers on healthcare compliance and preventive services, promoting overall health and well-being.
Coordinate follow-up care with external providers and vendors, ensuring continuity of care plans.
Utilize strong communication skills to foster effective relationships with enrollees, families, and healthcare providers.
Maintain professionalism and uphold a customer-focused approach in all interactions.
Qualifications3-5 years of experience in managed care and/or care management.
Background in clinical or community resource settings preferred.
Excellent verbal and written communication skills.
Bilingual proficiency in Spanish preferred.
Familiarity with medical terminology and computer systems, including Microsoft Office.
Strong organizational abilities and attention to detail.
Ability to make informed decisions under pressure while maintaining a positive attitude.
Willingness to participate in professional meetings and collaborate with multidisciplinary teams.
License/CertificationsLicensed in DC as a Registered Nurse (RN).
Certification as a Case Manager preferred.
Required EducationAssociates degree required; Bachelor's degree preferred.
Additional RequirementsAbility to manage mental and visual demands associated with PC use.
Flexibility to work occasional weekends, evenings, or early hours as needed.
