Job SummaryWe are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in managing a diverse panel of patients under the guidance of an addiction medicine provider. As a key member of our team, you will conduct comprehensive psychosocial assessments, develop personalized treatment plans, and provide ongoing case management and counseling services.
Key ResponsibilitiesConduct thorough psychosocial assessments and collaborate with medical providers to develop tailored treatment plans.
Coordinate care among patients, healthcare providers, and community services to ensure integrated, patient-centered care.
Utilize evidence-based interventions such as motivational interviewing, behavioral activation, problem-solving treatment, and cognitive behavioral therapy in individual and group settings.
Facilitate group programming for patients with co-occurring disorders and support their progress through treatment.
Document all patient interactions and care coordination activities in the electronic health record (EHR) and provide updates to the multidisciplinary team.
Research and recommend referrals to community resources and additional treatment programs based on patient needs.
Track treatment outcomes and participate in quality improvement initiatives to enhance patient care.
Administrative ResponsibilitiesLead staff training sessions to enhance clinical competencies and ensure compassionate care delivery.
Participate in weekly team huddles and contribute to the overall patient care strategy.
Required QualificationsLCSW with a Master's Degree in Social Work from an accredited program.
Florida state licensure (FL licensed).
Minimum 1 year of direct clinical experience working with individuals diagnosed with addiction, including both individual and group therapy.
Proficiency in Motivational Interviewing, Behavioral Activation, Problem-Solving Treatment, and Cognitive Behavioral Therapy.
Ability to work flexible hours, including early evenings (up to 7 pm).
Preferred QualificationsBilingual proficiency in English and Spanish.
Experience in population health management and the use of registry-based tools.
Interest in program development and quality improvement initiatives.
