Get It Recruit - Healthcare is Hiring a Remote LPN Chronic Care Manager - Remote | WFH

Position OverviewThe Chronic Care Manager plays a vital role as a Licensed Practical Nurse (LPN), supporting patient-centered, team-based care within primary care practices. Operating remotely, this role focuses on managing a panel of chronic care patients using the Phamily platform.
Key ResponsibilitiesDevelop a deep understanding of primary care practice requirements for effective population health management.
Collaborate closely with care teams to implement pre-visit and point-of-care processes tailored to specific patient populations.
Utilize the Phamily Chronic Care Management platform to coordinate care for multiple chronic disease patients across the care continuum.
Contribute to quality improvement initiatives and participate in care redesign efforts for population health.
Manage patient registries and provide necessary health data to support care team members.
Assist practice staff in developing proactive interventions for managing targeted patient populations.
Ensure a positive patient and family experience through professional and compassionate telephone and digital interactions.
Identify and report data inconsistencies promptly to ensure accurate information management.
Regularly participate in meetings with coworkers and practice staff to optimize care coordination efforts.
Adhere to all applicable federal, state, local, and company policies and procedures.
RequirementsRequired Qualifications:
Current LPN license with 5-10 years of nursing experience. Experience in population health is preferred.
Proven ability to solve problems and manage multiple tasks effectively.
Excellent written and verbal communication skills.
Strong customer service orientation.
Preferred QualificationsPrior experience with Electronic Health Records (EHR/EMR) systems.
Previous Care Management experience with patients having chronic health conditions.
Bilingual proficiency in English and Spanish is a plus.
BenefitsFull-time position with a salary of $22/hr.
Eligibility for 401K after one year of tenure.
Paid Time Off (PTO) and Paid Holidays.
