Role DescriptionYour Guiding Star:
Our focus is on providing exceptional personalized support to our patients, emphasizing regular engagement, proactive identification of medical needs, and facilitating connections to community-based services to address social determinants of health (SDOH).
We are seeking a compassionate team member to offer virtual support to our pregnant and postpartum patients. In this role, you will collaborate closely with an interdisciplinary care team, serving as the primary point of contact for perinatal patients. Your main responsibilities will involve engaging with patients, establishing trust, identifying their needs, and connecting them with appropriate care to enhance their well-being. Building supportive relationships with patients over extended periods, you'll provide support via text, phone, or video calls. This position is full-time, salaried, with working hours from Monday to Friday, 10 am to 7 pm ET.
In This Role, You WillCollaborate with patients to establish trust, identify goals, increase engagement in their healthcare journey, address barriers, and ensure adherence to their care plan.
Facilitate connections between patients and community-based resources, empowering them to address various psychosocial needs with local support services.
Conduct virtual visits in collaboration with our OB providers.
Proactively reach out to and follow up with patients virtually to ensure they receive the necessary care and support.
Work alongside the multidisciplinary care team to ensure comprehensive medical and social care throughout the perinatal journey.
Minimize care gaps through regular engagement with patients and internal referrals to clinicians as required.
Maintain accurate documentation of all patient interactions and escalate issues to the care team when necessary.
Contribute to the development of programs and products by providing user feedback.
RequirementsWho You Are:
You possess a minimum of 4 years of healthcare experience in either inpatient or outpatient OB settings, such as a medical assistant, nursing assistant, inpatient unit secretary/coordinator, or equivalent role.
You have experience working with perinatal patients who have experienced pregnancy-related conditions such as diabetes, hypertension, or perinatal loss.
Excellent customer service skills and strong, empathetic communication abilities are integral to your approach.
You are internet-connected and capable of working remotely via video, phone, and text.
You have extensive experience navigating local community resources and programs.
Your care approach is characterized by empathy, cultural humility, and sensitivity to diverse patient needs.
Strong interpersonal, relationship-building, and communication skills are among your strengths.
You possess strong attention to detail and can effectively prioritize tasks.
Proficiency with email, spreadsheets, and a willingness to adapt to new software are essential.
You are a flexible and adaptable thinker who embraces change readily.
Bonus Points If YouHave experience providing virtual care or working in a fast-paced environment, such as a high-growth company or startup.
Are familiar with using Athena as an EMR.
Are fluent in both Spanish and English.
