What You'll Be DoingAs a Patient Care Coordinator, you'll collaborate with clinical teams and our Case Management team to support patients in managing their healthcare journey effectively. Your responsibilities will include:
Remote engagement with patients and medical practice staff.
Independently managing a targeted patient population while closely collaborating with medical practices.
Assisting patients in addressing health concerns, navigating care, and implementing personalized care plans.
Documenting patient medical history accurately in electronic health records (EHR).
Building trusting relationships with medical practices and within our organization.
Meeting Key Performance Indicators to enhance patient health outcomes and reduce medical costs.
Utilizing various communication modalities to engage high-risk patients effectively.
Escalating medical issues as needed and ensuring compliance with security policies to protect patient information.
What You'll Need To HaveWe're looking for candidates with:
Passion for patient coordination and serving individuals with complex medical and social needs.
Empathy and proficiency in motivational interviewing and behavioral change management.
Strong Conversation Skills And Precise Documentation Abilities.Knowledge of medical terminology and primary care physician practices.
Understanding of social determinants of health; bilingual (English and Spanish) skills are highly desired.
Certification as a Medical Assistant (preferred) or equivalent experience in healthcare roles.
Familiarity with electronic health records (EHRs) and care management databases.
Ability to multitask, prioritize, and work independently in a remote environment.
Excellent written, verbal, and visual communication skills.
