Key ResponsibilitiesEngage with assigned patients through phone calls, building trust and relationships to guide them through their healthcare journey.
Enroll patients in specialized programs tailored to their chronic conditions or recent healthcare events.
Ensure patient adherence to care plans and medication regimens.
Coordinate closely with aligned practices for timely visits, follow-ups, and medication refills.
Perform comprehensive assessments, considering physical, social, and psychological aspects, and collaborate with multidisciplinary teams for holistic care.
Identify care gaps and opportunities for intervention, collaborating with providers and healthcare teams for seamless transitions and optimal outcomes.
Provide insights and recommendations for patients eligible for hospice and palliative care.
Implement member-centric care plans with SMART goals, continually evaluating effectiveness and making necessary revisions.
Maintain accurate documentation and adhere to regulatory standards and policies.
RequirementsEducation:
Associates degree in nursing required; BSN preferred.
Licensure/CertificationMust hold a current/active FL compact RN license.
ExperienceMinimum two years of nursing-related care experience, preferably in a primary care office or call center setting.
Experience in case management, care coordination, or disease management preferred.
Location & Shift:
Remote work initially, with potential office presence in Orlando, FL.
Shifts may vary.
Knowledge, Skills, And AbilitiesProficiency in care management concepts along the continuum of care.
Advanced clinical knowledge of chronic disease states.
Skills in motivational interviewing and conflict management.
Strong communication skills, both written and verbal, with the ability to articulate complex concepts clearly.
Ability to manage multiple demands effectively and think critically within the nursing scope of practice.
