Job SummaryWe are currently seeking a compassionate and dedicated Social Worker to join our Cardiac Rehabilitation Team. Reporting to the Cardiac Rehabilitation Manager, you will play a vital role in supporting participants, their families, and significant others as they navigate the educational and exercise components of heart disease. Your responsibilities will include integrating these components with a supportive process that addresses emotional, family, vocational, and stress management needs.
EducationMinimum Qualifications:

ExperienceMaster of Social Work (MSW) degree
Licensure And/or CertificationExperience in counseling patients is required
LICSW Preferred, LCSW RequiredBLS Healthcare provider certification required
SkillsExcellent customer service skills
Proficiency in English language (reading, writing, communication)
Basic Computer Skills RequiredAbility to maintain confidentiality of sensitive information
