Get It Recruit - Professional Services is Hiring a RN Care Manager for High-Risk Patient Support Near Oklahoma, OK

DescriptionWe are looking for a compassionate and dedicated RN Care Manager who will focus on supporting high-risk members with chronic behavioral and/or health conditions. In this role, you will collaborate closely with members, caregivers, physicians, and the healthcare team to ensure timely access to care, continuity across settings, and informed decision-making. Additionally, you will assist in connecting members to supportive services and community resources.
This position involves managing all aspects of care, including palliative care, aimed at improving health outcomes for members. The role is designed for a full-time remote work environment based in the Oklahoma City area, with occasional travel to Tulsa for meetings. Care management will be conducted through various methods, including telephonic communication, home visits, and clinic or hospital visits.
Key Responsibilities
  • Collaborate with members to create personalized care plans, identifying goals, barriers, and strengths.
  • Perform clinical assessments of medical, behavioral, and social health needs, identifying barriers to care.
  • Conduct ongoing medication reconciliations and assess for potential obstacles to care.
  • Monitor and evaluate care plans, track progress toward goals, and ensure member satisfaction.
  • Interpret medical test results and evaluate treatment effectiveness.
  • Recognize and communicate changes in member health status.
  • Provide various methods of contact with members based on the care plan.
  • Conduct home visits to evaluate environments and assess safety concerns.
  • Facilitate communication and collaboration between members and healthcare providers for coordinated care.
  • Assist members in transitioning between care settings to ensure continuity of care.
  • Document care management activities and maintain accurate records.
  • Participate in interdisciplinary case reviews and quality improvement activities.
  • Educate and advise members and caregivers on conditions, treatments, and self-management.

Qualifications
  • Education: Bachelor's Degree in Nursing preferred; Associate's Degree in Nursing with 2 years of professional nursing experience will also be considered.
  • Licensure: Current Registered Nurse (RN) certification in the state of Oklahoma.
  • Experience: Minimum of 1 year of professional nursing experience is required for candidates with a Bachelor's Degree.
  • Skills:
    • Strong knowledge of case management principles and medical terminology.
    • Ability to resolve complex issues using critical thinking and problem-solving skills.
    • Excellent organizational skills with the ability to prioritize and complete tasks in a timely manner.
    • Ability to work under pressure and manage stress.
    • Strong interpersonal skills to build and maintain professional relationships.
    • Self-motivated, flexible, and open to feedback.
    • Proficiency in maintaining accurate, detailed records.
Working Conditions
