Job Title: Physical Therapist Assistant
**What You’ll Do – Job Responsibilities**
As a Physical Therapist Assistant, you will be integral to implementing an innovative integrated healthcare approach that enhances patient care while fostering a strong provider-patient relationship. Your focus will be on delivering exceptional physical therapy services and movement programs that improve health outcomes and patient engagement. Under the guidance of a licensed physical therapist, you will execute treatment plans, deliver group classes, and support the clinical team in providing high-quality care. Your core responsibilities will include, but are not limited to:
- Administer routine physical therapy treatments as directed by a licensed physical therapist.
- Develop and document personalized home exercise programs for patients, ensuring effective patient education.
- Maintain comprehensive documentation of treatment sessions and patient communications.
- Organize and facilitate weekly group movement and physical activity classes tailored for patients, including fall prevention and chronic disease management.
- Offer virtual physical therapy sessions and in-home visits for high-risk patients when necessary.
- Educate patients on chronic disease management, emphasizing the importance of movement and physical activity for prevention and treatment.
- Collaborate with healthcare providers, interdisciplinary teams, patients, and caregivers to establish and pursue individualized patient goals.
- Participate in quality improvement initiatives aligned with clinical priorities.
- Leverage data to drive the implementation of physical therapy programs that support established healthcare metrics, such as HEDIS/Star measures.
- Ensure a positive patient experience through timely, professional, and empathetic service.
**What You’ll Bring – Education Requirements**
- Graduated from a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited program.
- Active license from the Texas Board of Physical Therapy Examiners or the ability to obtain one.
- Current Basic Life Support (BLS) certification.
**What You’ll Bring – Experience Requirements**
- A minimum of 2 years of experience as a Physical Therapist Assistant is preferred.
- Bilingual proficiency in English and Spanish is required.
- Experience in leading group physical activity classes is preferred.
- Previous experience working with geriatric populations is an advantage.
- Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint.
