**Responsibilities**
Provide support to the clinical team and act as a proactive resource for members concerning disease prevention and education on available benefits and services.
Conduct educational phone calls with members to explain programs, complete health needs assessments, and refer them to relevant population health management programs.
Reach out to members who have been identified with gaps in care, encouraging compliance, and assisting them in scheduling appointments with providers.
Manage system work queues by screening members for eligibility and reviewing prior activities, claims inquiries, and authorization history, while assigning members to the clinical team for further outreach.
Facilitate research and analysis on inquiries or complaints related to processes and member incentives.
