Summary: The Hospice Intake Coordinator is responsible for managing all aspects of the patient intake process including collaboration with all the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
Job Duties: 1.Obtains all pertinent documentation necessary for admission: History and Physician, Insurance info, Labs, Med List, Order, etc.
2. Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake.
3.Coordinates with Medical Director/Nurse Practitioner to schedule initial palliative or hospice consult visitwith patient, as necessary.
4.Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
5.Establishes and maintains positive working relationships with current and potential referral sources, internal and external.
6.In cooperation with DON/CSM, ensures seamless transition of patients to hospice by providing telephonic patient/family education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
7.Gathers, collates, and reports referral statistics including key customer referral trends. Tracks weekly/monthly referral numbers.
8.Maintains an accurate listing of current daily census including activities related to admissions, discharges, pending referrals and transfers.
9.Identifies opportunities for additional or improved services to meet unmet customer needs.10.Maintains comprehensive working knowledge of the hospice contractual relationships and ensures that patients are admitted according to contract provisions. Assists with maintaining facility and hospital contracts/files.11.Completes all job duties in a manner that meets the accepted standards of practice and the La Diferencia Hospice Policies and Procedures.
12.Contributes to achieving the hospice's mission.
13.Works collaboratively with others to achieve goals.
14.Coordinates information sessions, legals, and admissions schedule with DON, CSM and ED.
15.Other duties as assigned.
Qualifications: - 2 years of previous experience with intake and referral management
