Job Summary:

Uses the nursing process to plan, implement, direct, and evaluate patient care to achieve optimal patient outcomes; works collaboratively with all other disciplines on assigned unit.

Essential Job Duties:

  1. Conduct initial and ongoing age-specific assessments in designated time frames.
  2. Contribute to Interdisciplinary Plan of care using assessment data.
  3. Evaluate effectiveness of plan/of care and modifies, as appropriate.
  4. Involve patient/family/significant other in developing plan of care.
  5. Collaborate with other clinical disciplines in developing plan of care.
  6. Provide care based on scientific principles/nursing theory and evidence based practice.
  7. Recognize change in the patient’s condition and intervene appropriately.
  8. Prioritize nursing actions, reflecting patient needs, acuity, and desired outcomes.
  9. Implement physician orders accurately within appropriate time frames.
  10. Demonstrate competency in core & unit-specific clinical skills.
  11. Identify continuing care needs of the patient for the discharge plan.
  12. In the absence of a front desk coordinator, scheduling patients for HBO therapy or wound care visit, utilizing hospital software/ systems as well as RXH’s EMR system may be required.
  13. In the absence of a front desk coordinator, registration to be accomplished and in accordance with hospital policy and software / systems requirements as well as RXH’s EMR system.
  14. All patient treatment notes to be entered into RXH’s EMR system.
  15. Provide patient/family education to include:
    • Assessment of learning needs, abilities and readiness to learn,
    • Documentation of teaching activities,
    • Provision of instruction as appropriat e.g. painmanagement, medication, diet, equipment, activities after discharge
    • Provision of appropriate educational materials, as available prior to discharge.
  16. Document all care provided accurately & completely according to Hospital and Department Policy.
  17. HIPAA Compliance – complies with all of the Privacy and Confidentiality Policy and Procedures outlined in the “Privacy, Security and Confidentiality Agreement.
  18. Adheres to all Patient Safety Initiatives and Infection Control Procedures as outlined in the hospital’s safety manual. Appropriate use of Hospital resources, supplies and equipment.
  19. Process and utilize all computerized patient care functions, e.g. ADT, diagnostic test results, etc.
  20. Apply for nursing council or committee and be vested on one council at least every 5 years. 80% attendance in person or by phone is required. Advancements in Clinical Ladder will be reflected in yearly application process and is expected for job performance standards

Secondary Job Duties That May be Reassigned:

  1. Completes other duties and special projects as assigned.
  • Must have and maintain a valid CPR certification from the American Heart Association.
  • Must have and maintain a valid Nurse Practitioner (NP) or Registered Nurse (RN) license in the state in which the Nurse will work; Bachelors in Nursing within 3 years of employment (with the exception of staff previously granted grandfather status).
  • Must have prior experience working with wound care patients.
  • Prior experience with billing and coding preferred.
