Pay Rate: $19-$22 an hour Job Description: The Intake Coordinator is responsible for providing transformative coaching to clients by identifying, meeting, and achieving their goals towards stabilization by coordinating wrap-around services between internal/external divisions, agencies, and/or providers. The Intake Coordinator addresses the physical, psychological and social needs of the family, by creating a strong working relationship. They will always need to remain professional and understanding. The Intake Coordinator will support the client with accessing the services and resources necessary to remove poverty and barriers to stability. 1. Providing support by contacting clients on a weekly basis regarding services and supports 2. Making individual/family recommendations and referrals 3. Assisting clients with identifying and achieving measurable goals 4. Evaluating progress to attain goals and providing guidance to revise Family Stabilization plans, as needed 5. Offering educational and vocational services and referrals to help support sustainability 6. Supporting clients by advocating for their needs and working with service providers and community partners 7. Helping to manage crisis 8. Keeping accurate records of calls, visits, and business conducted on behalf of the client 9. Strong commitment to helping families improve their living and keeping their information confidential 10. Providing weekly updates on clients at Dual Generation Center meetings 11. Working with stakeholders to provide the best possible service plan to Dual Generation Clients 12. Supporting community partnerships and events 13. Maintaining efficient, confidential, and organized records in the organizational database to ensure accurate recordkeeping Minimum Qualifications: 1. Ability to be flexible, work independently and collaboratively 2. Ability to multitask, manage details and meet deadlines 3. Excellent communication and interpersonal skills 4. Current Driver’s License with acceptable driving record 5. Ability to interface effectively with professionals from outside agencies (excellent verbal/written communication skills) 6. Knowledge and understanding of flexible, diverse, traditional, and non-traditional support, advocacy and intervention processes 7. Proven ability to establish an excellent rapport with staff, clients, and partners 8. Strong organizational and administrative skills including completion of monthly reports coupled with proficient soft-ware computer skills 9. Demonstrated proficiency in crisis intervention, conflict resolution, and mediation 10. Effective problem solving, advocacy and assessment skills 11. Proficiency in computer skills, i.e. Microsoft office, Google Drive, and other databases 12. College degree preferred. In lieu of a degree, a minimum of 2 years’ experience in case management
