Summary
Position Summary
Maternal Health Advocate (MHA) is responsible for the coordination and implementation of patient screening of perinatal and postpartum mental health conditions, and social determinants of health (SDoH). This position provides direction and guidance for the patient by leading discussions on the patient's postpartum experiences, reviewing SDoH needs, and provide resources, supportive guidance, and supportive networks for postpartum mental health and socioeconomic barriers. The MHA will need to develop and maintain effective partnerships with various community agencies, physicians and other Healthcare professionals.
Position Qualifications
Minimum Education: High School Diploma/GED.
Minimum Experience: 1 year serving in Healthcare, Community or Public Health
The ideal candidate would be someone with previous experience working in the public health space. Someone with lived experience who is familiar with the community.
