Work Schedule:
- Full-time position: Monday to Friday, 8:00 a.m. to 5:00 p.m., with occasional extended hours until 8:00 p.m.
- Weekend event participation may be required.
- Average workweek of approximately 40 hours.
- Perform comprehensive health assessments and screenings for individuals and families.
- Diagnose and manage acute and chronic medical conditions, emphasizing preventive care and health promotion.
- Collaborate with community organizations to develop and implement health education initiatives.
- Advocate for patients' rights and healthcare accessibility by providing culturally sensitive care.
- Coordinate referrals and ensure seamless follow-up care for patients.
- Maintain accurate and compliant medical records in accordance with regulatory requirements.
- Engage in community outreach events and health fairs to promote awareness and provide health screenings.
- Stay informed about community health trends, evidence-based practices, and relevant policies.
- Deliver confidential and comprehensive healthcare services with empathy and respect for all patients.
- Uphold the highest standards of care quality.

- Current Nurse Practitioner certification and active state nursing license.
- Master's degree in Nursing from an accredited institution.
- Minimum of 3 years of clinical experience, preferably in community or public health settings.
