We're seeking a detail-oriented and compassionate MDS Coordinator to join our vibrant team! In this crucial role, you'll play a vital part in ensuring our residents receive the highest quality care possible.

Key Responsibilities :

• Coordinate and oversee the completion of MDS assessments in accordance with federal and state regulations.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Monitor and evaluate resident care plans to ensure they meet the needs and preferences of each resident.
• Provide education and training to staff on MDS processes and documentation requirements.
• Participate in quality improvement initiatives to enhance resident care outcomes.
• Maintain up-to-date knowledge of MDS regulations and best practices.

Qualifications :

• Valid RN or LPN / LVN license in MINNESOTA .
• Minimum of 2 years of experience as an MDS Coordinator (or related field) in a long-term care setting.
• Comprehensive knowledge of MDS 3.0 guidelines and regulations.
• Strong analytical and organizational skills.
• Excellent communication and interpersonal abilities.
• Proficiency in electronic health records (EHR) systems.
• Ability to work independently and as part of a team.
