The person in this position will provide direct casework and counseling services to the patient/family as directed by the plan of care. The social worker provides psychosocial assessments and develops a corresponding care plan as coordinated with the Interdisciplinary Team
Job Description:
- Provide psychosocial assessments, direct casework, and counseling services to patients and their families
- Participate in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished
- Work collaboratively as a member of the Interdisciplinary Team(s)
- Prepare clinical and progress notes for the clinical record per Hospice, state, and federal guidelines/standards
- Prepare documentation for the clinical record per Hospice, state, and federal guidelines and standards
- Maintain and update knowledge of community resources. Facilitate referral of patients/families to those resources as needed and desired
- Participate in taking on-call responsibilities as necessary including weekends and evenings
- Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested
- A Master’s degree from an accredited educational institution of social work is preferred or another related clinical degree
- Must be a licensed social worker
- Strong interpersonal and communication skills are required
- Experience in health care and clinical social work is preferred
- Knowledge of the process of death, dying and bereavement is required
- Understanding of psychosocial factors operative in terminally ill patients and families required
- Unlimited access to a reliable car
- Current state driver’s license and auto insurance
