As a Nurse- RN/LPN and a valuable team member, you will ensure that the needs of the residents are met in a timely and dignified manner. This position requires a current PA RN or LPN License.

About Us:Founded in 1935, the Community at Rockhill (www.communityatrockhill.org) is a residential community for adults 65 and older located in Bucks County, Pennsylvania. As a Life Plan Community, Rockhill offers a continuum of care that includes Residential Living, Personal Care for adults who need assistance with daily activities, and Health Care for individuals in need of 24-hour support.
Guided by God's love, Grace Inspired Living is dedicated to providing communities and services that enhance the lives of older adults in a supportive setting that values residents, employees, volunteers, and families.
SPECIFIC DUTIES AND RESPONSIBILITIES:Report at start of shift with worksheet to receive shift report.
Assume leadership by coordinating the care of residents with appropriate personnel.
Count narcotics with off-going nurse.
Administer scheduled medications in accordance with facility, state and federal regulations. Perform scheduled treatments, wound assessments and documentation as per facility policy.
Document pertinent information on residents including but not limited to: 7 day assessments for MDS documentation, alert charting, monthly summaries, incidents/accidents and vital signs.
Participate in admission assessments, transfers, deaths, physician orders and prepare for physician rounds.
Regular attendance is an essential function.
Communicate resident or staff issues to appropriate personnel.
Attend meetings and in-services as required.
The 11p - 7a nurse is responsible for the neighborhood medication sheet re-caps.

The dedicated staff ensures that residents enjoy the best quality of life while maintaining the highest possible level of independence. Telford consists of 245 independent living units, 125 personal care beds in 97 units, and 75 skilled nursing beds.

- Graduate of an approved school of practical or vocational nursing.
- Current PA LPN/LVN or RN license.
- Current CPR certification.
- Experience is preferred in rehabilitative or geriatric nursing.
