GRAND Mental Health is Hiring a Mental Health Co-Response Case Manager Near Tulsa, OK
Interviews and assesses patients and/or patients family, caregivers, and /or legal representatives.
Determines prioritizes and provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor and evaluate compliance with patients treatment plan.
Responds in the field as a co-responder with local law enforcement to patients in active crisis.
Will be required to assess and plan interventions to help patients cope with social, emotional, economic and environmental problems. Requires a Case Manager II Certification and will work with local area (Washington County) first response agencies. This position will work non-traditional hours and schedule will be created based on call volume. Bachelors Degree and/or ability to obtain Case Management II Certificate. Must have a valid Oklahoma Drivers license. Must have reliable transportation and proof of insurance. An expectation of core competencies in relation to individuals with co-occurring disorders.
