Grand Valley Nursing is a Home Health Agency that services the Cody, WY area. We are professional, agile, collaborative and our goal is to provide the community with 8-, 12-, and 24-hour In-home Healthcare to meet their needs.

- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and make necessary adjustments to the care plan
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Collaborate with other healthcare professionals to ensure coordinated and effective patient care -Transportation to clients' Physician, pharmacy, and daily errands -Homemaker Services - Proper Documentation each shift in the communication log including all delgated tasks
- Document all patient interactions, assessments, and interventions in the commuication log
- Adhere to infection control protocols and maintain aseptic technique during procedures

- Valid RN license in the state of Wyoming
- Proficiency in foley care, wound care, blood draw, IV insertion and medication administration
- Strong assessment, critical thinking, and problem-solving skills
- Ability to provide compassionate and patient-centered care
- Excellent communication and interpersonal skills
- Knowledge of infection control protocols and aseptic technique
- Understanding of home care systems and regulations
