The Granite VNA Home Care Home Health and Hospice Aide performs personal care, designated health-related services for client's in their homes under the supervision of a Registered Nurse. The Home Care Home Health and Hospice Aide interacts with client’s family members, facility staff and other healthcare team members while maintaining professional standards.

Essential Job Functions
Follows care plan in providing basic care and services to the client including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff clearly, concisely and in a timely manner.
Documents care provided to the resident accurately in accordance to the agency’s guidelines.
Complies with the Agency visit documentation, time entry and electronic scheduling requirements.
Communicates clearly and courteously with others.
Practices safe work habits and proper body mechanics.
Arrives on time to visits and completing visits within the scheduled time allowed.
Other Job Functions
Assists client with homemaking tasks, medication assistance, and wound care according to the patient's care plan.
Performs other duties as assigned.

Requirements

Minimum Qualifications and Requirements
High school diploma/GED, accompanied by current New Hampshire LNA license
Reliable automobile 
Automobile Insurance
Valid Driver’s license
Ability to meet all Post-Offer Pre-employment requirements (including physical capacity exam, background checks, etc.)
If hired, be available Monday-Friday for the first weeks of employment to complete the required training.
Ability to understand and carry out directions and instructions
Ability to work with minimum supervision
Ability to read, write and speak English
