Greater Flint Health Coalition is seeking an Program Director, to join our Mid-Michigan Community Health Access Program (CHAP) team.

Overview:Greater Flint Health Coalition (GFHC) is seeking an experienced administrator and programmatic leader to serve in the position of Program Director of the Mid-Michigan Community Health Access Program (CHAP).

Mid-Michigan CHAP was launched in 2015 as a community-based population health and care management program serving a diverse network of healthcare providers, physician organizations, health plans, and hospitals to collaboratively address the clinical and community needs impacting residents, patients, and beneficiaries, and their ability to both access health care and achieve optimal health outcomes. Mid-Michigan CHAP seeks to connect residents to a medical home, remove barriers to care typically associated with the social determinants of health, while raising the quality of care, better utilizing resources, and decreasing costs. Mid-Michigan CHAP’s priority population is children and adults covered by Medicaid insurance, as well as those residents who are uninsured and underinsured.

The Program Director leads a multidisciplinary team of licensed social workers, registered nurses, and certified community health workers, which engage and connect clients to medical homes (for both sick visits, well visits, and preventive care) and provide navigation support related to insurance enrollment, public benefit programs, and other community-based agency and social service resources to address physical and mental/behavioral health needs, health education, transportation, food insecurity, financial assistance, and other presented social and resource related barriers to care. The Program Director leads collaborative engagement with physician practices, physician organizations, health insurers, hospitals, state/local government agencies, and community-based agencies to maintain and expand the integrated service delivery and referral process led by Mid-Michigan CHAP. The Program Director will also provide administrative leadership to targeted programs serving specialized populations; for example, Mid-Michigan CHAP is the state-designated provider of elevated blood lead level nurse case management services in Genesee County and is also a provider of Targeted Case Management services as a component of the CMS-approved Flint Medicaid Expansion Coverage Program.

The Program Director will join our team based in Flint, Michigan, working in a fast-paced, outcome driven, collaborative environment. GFHC serves as the neutral convener and backbone organization for public health and healthcare collaboration between Flint & Genesee County hospitals, physicians, insurers, businesses, government, public health, education, nonprofits, and residents. GFHC’s mission is to improve the health status of area residents, and to improve the quality and cost effectiveness of the healthcare systems in the community.

Primary Responsibilities:

Provide leadership and administrative structure to Mid-Michigan CHAP to achieve programmatic outcomes related to GFHC’s strategic objectives, grant and health plan contract deliverables, partnership agreements, and program work plans.

  • Lead and direct CHAP managers and collaborate with GFHC administrative leadership and program stakeholders to design, implement, and achieve CHAP program outcomes.
  • Evaluate and report performance metrics and outcomes related to strategic program goals associated with client service and engagement, partner referrals and feedback, and the collective outcomes of CHAP.

Oversee, structure, support and ensure excellence in program operations.

  • Oversee the CHAP referral/intake system to optimize efficient CHAP caseload, referral monitoring, intervention strategies, and staff assignments.
  • Manage project timelines, documentation, and deliverables while maintaining alignment with multi-sector partner objectives and grant / contract work plan requirements.
  • Oversee the development and maintenance of CHAP operations, training and support manuals, building a culture of compassion and excellence.

Direct and develop management personnel, care coordinators, and community navigators to engage residents in a consistent, respectful, and effective manner.

  • Work with Human Resources and other GFHC personnel to recommend, recruit, and evaluate personnel.
  • Directly supervise CHAP managers and program staff, including subordinate managers, social workers, nurses, community health workers, and administrative staff. Support staff efforts to become licensed and certified.
  • Oversee day-to-day staffing ratio, training plans, and client engagement outputs.
  • Promote teamwork and good communication among all staff / team members within the workflow of CHAP and the GFHC.
  • Ensure client engagement staff and care coordinators engage residents in a culturally competent, respectful, and compassionate manner in all interactions.

Lead CHAP partnerships with referring medical practices, health plans, and multi-sector community stakeholders.

  • Work in partnership with GFHC leadership and the CHAP Medical Director to plan and execute Steering Committee, Provider Workgroup and advisory committee meetings.
  • Work closely with physicians, practice managers and referral coordinators of pediatric and family medicine practices to orient, train, and reinforce their use of the support services CHAP provides to drive continuous monthly referral volume.
  • Develop, implement, and maintain practice engagement strategies to ensure referral outcomes and feedback, satisfaction, quality improvement and client services initiatives meet objectives. Identify problems which may arise and coordinate problem solving collaborations.
  • Maintain good communication and support teamwork with and among the practice managers’ network; organize and manage the CHAP practice manager’s meetings, determining issues to be discussed, overseeing all preparations, running the meetings and documenting outcomes.
  • Lead programmatic committee planning and staffing, ensuring responsiveness to partner requirements, inputs, and concerns. Collaborative process includes timely agenda development, minutes, program/grant development, work plan execution, outcomes reporting, and the completion of required committee follow up resulting from consensus decision making.
  • Recommend, recruit, work effectively with and hold accountable program consultants and advisors.

Manage, develop, and sustain health plan and contractual agreements supporting CHAP.

  • Manage contracts and ensure compliance and success is achieved relative to contracts, business associate agreements (BAAs), and grant agreements with Medicaid Health Plans, health care and practice partners, health departments, and grant funding entities.
  • Develop new partnerships and contracts specific to CHAP’s strategic business plan objectives and service growth model.
  • Work with GFHC administration in writing and managing program contracts grants, and ensuring all reporting is completed and submitted timely.
  • Collaborate with GFHC administration on programmatic budget and staffing ratios to maintain financial and programmatic capacity needs and sustainability.

Engage in program development and organizational advocacy.

  • Participate in advocacy efforts for improved access and quality of health care for low-income and uninsured (or underinsured) children and adults, by attending selected governmental agency meetings, legislative sessions/meetings, community advisory boards, and other community organization meetings.
  • Develop and maintain good working relationships with key partners at selected state, county, and community organizations and agencies.
  • Represent Genesee County as a member of the Michigan CHAP (MI-CHAP) network and connect with other statewide and national patient centered medical home, care management, and social determinant of health initiatives as appropriate.

Engage with and advocate for the success of the GFHC’s strategic business plan.

  • Support the mission, vision, values and key strategies of the Greater Flint Health Coalition, and prioritize the development of collaborative relationships with the organizations that interface with CHAP.
  • Report to ­­the Chief Executive Officer and participate in required Greater Flint Health Coalition cabinet meetings; serve on pertinent committees and task forces as assigned including the Access Committee, Genesee Community Health Innovation Region Steering, and Community Referral Network.
  • Participate and support the development of other projects and efforts that correlate or impact CHAP.

Qualifications & Experience Required:

To be successful in this position, a candidate must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  • Master’s degree in healthcare, public health, education, or related field. Master’s degree in Social Work and Licensed Masters Social Work certification (LMSW) strongly preferred.
  • Five or more years’ experience leading and managing care management and/or community health improvement programs.
  • Five or more years’ experience supervising programmatic or clinically licensed staff; preference to candidates with a track record supervising licensed social workers, registered nurses, certified community health workers, or some combination of these classifications of distinguished professionals.
  • Motivated self-starter that is career-oriented and driven to make a difference, not seeking “just a job”.
  • Current unrestricted Michigan Driver’s License, automobile insurance, and available automobile.
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, budgets, and legal documents.
  • Ability to respond to inquiries or complaints from customers, regulatory agencies, or members of the community.
  • Ability to write well, speak well, present well, and use communication and communication methods appropriate to the situation.

Reports to: Chief Executive Officer

Job Status:

Full Time, Salary; Monday through Friday, 8:30 a.m. to 5:00 p.m. with meetings and events earlier, later, or on weekends as required by the responsibilities of the position.
