This position serves in an expanded nursing role that collaborates with the medical team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. The person in this role integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric and culturally sensitive, promoting quality and efficiency in the delivery of healthcare. The Nurse Care Manager – MCH provides professional nursing care to maintain and promote the health of Greater Portland Health (GPH) patients utilizing the nursing process.
Care Management: 60% Direct Patient Care: 40%

• Registered Nurse with one to two years of Home Care, Ambulatory Care, or Acute Care experience preferred.
• Licensed at an RN level by the State of Maine Board of Nursing is required.
• Bachelor’s degree in nursing preferred.
• BLS certification must be current.
• Experience in care management/care coordination to include care planning, resource management, and utilization management desired.
• Prior experience in primary care setting desired.
