Greater Springfield Senior Services, Inc. (GSSSI) is Hiring a Community Transition Liaison Program (CTLP) Case Assistant, Bilingual English/Spanish AND non-bilingual positions Near Springfield, MA
Newly created position in NEW Program! Full-time, 35 hours/week NOT Remote Responsible for assigned clerical administrative functions for the Community Transition Liaison Program (CTLP)
Accurately process, maintain and archive departmental and consumer records via manual and/or computerized means in accordance with program/system requirements
Maintain working knowledge of all relevant State Home Care (SHC) and CTLP databases
Supports the Community Transition Liaison to assist consumers through discharge from Skilled Nursing Facilities (SNFs) back to the community
Accompanies the Liaison on Nursing Facility visits as needed
Gathers documentation needed to assist the consumer in applying for public benefits
Assists the consumer in completing and submitting housing applications
Produce and maintain statistical/tracking data, and other specifications as directed
Work both independently as well as part of a team.
Education and Experience
High School Diploma.
1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting
Good computer skills including working knowledge of spreadsheets, word processing and data entry (including Windows, Word and Excel. Use table formatting in word processing and spreadsheet applications as needed
Must be able to operate a keyboard 80-90% of the time
Must be able to climb multiple flights of stairs Travel reimbursement is $0.62/mile
