  • · Must demonstrate knowledge & applicability of federal, state and local laws, regulations, and standards as they apply to the services we perform. This includes HIPAA security and privacy laws.
  • · Must maintain all required certifications, licenses, and / or accreditations as needed to fulfill job requirements.
  • · Must be willing to accept evaluation of services performed and make adjustments accordingly and in a timely manner.
  • · Must complete all mandatory and required trainings, on-time, upon orientation and annually thereafter.
  • · Must be willing to provide in-service education / training for others, if asked.
  • · Must be willing to participate in the agency’s Performance Improvement / Quality Assurance program.
  • · Most be able to use our electronic medical record system. This work is repetitive, requires long periods of sitting and/or standing and typing and must be done daily.
  • · Must be able and eligible to drive an agency vehicle, possess a valid driver’s license, current vehicle insurance and pass a MVR.
  • · Must be able to pass an OSBI background check.
  • · Must be able to move up to 50 pounds.
  • · Must be willing to perform other duties as requested by your immediate supervisor or members of the agency’s Leadership Team.
  • · Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
  • · Understands due to CMS mandate all GCBHS employees must be fully vaccinated against COVID19, unless an exemption for medical or religious exemption has been granted.
  • ·Must have a bachelor’s degree in a mental health or related field.
  • · Is certified as a Case Manager by the ODMHSAS, with the ability to maintain certification.
  • Essential Job Functions
  • · Reviews client charts to ensure all CCBHC outcome measures are being met in accordance to State and GCBHS guidelines.
  • · Monitors client charts to ensure appropriate services have been completed for PPS reimbursement.
  • · Monitor client charts for completion of all required documentation.
  • · Monitors client charts to ensure services are being provided according to the timeline guidelines.
  • · Accurately records all chart information and outcome data on spreadsheets that are easily read and understood.
  • · Coordinates on a regular basis with his/her direct supervisor regarding CCBHC outcomes.
  • · Facilitate huddles with team members on a daily or no less than twice weekly timeframe to coordinate client needs andoutcomes management.
  • · Monitor client charts to ensure all clients are receiving services according to their individual level of care.
  • · Interacts daily with clinicians to resolve questions, inconsistencies, or missing data.
  • · Meets with clients for case management or wellness services as required by the Team.
  • · Provides care coordination, comprehensive transitional care, individual and family support, and referrals to community and social supportive services to those we are serving.
  • · Assists those we serve to gain knowledge and skills to reduce problems and improve opportunities to lead healthy, self-sufficient productive lives.
  • · Understands that Care Coordination is the foundation of our service delivery model.
  • · Fosters individual responsibility for health awareness to those we are serving.
  • · Assists clients in the development of building natural support systems, to include family and community supports.
  • · Understands and can demonstrate knowledge of the primary functions of Integrated Care.
  • · Guides clients into the development of skills and strategies for dealing with their problems.
  • · Assists in the development and implementation of service plans and collaborates with other staff and clients in goal setting and discharge planning.
  • · Can demonstrate knowledge in the role trauma plays in recovery and provides trauma informed, gender sensitive, age appropriate, culturally competent, and person-centered treatment.
  • · Is committed to providing data-driven services and can articulate and demonstrate what that means.
  • · Can dialogue with the people we serve about their view of recovery and what that means to them and how to utilize those skills in their aftercare.
  • · Can effectively work with primary care providers for the integration of care.
  • · Can demonstrate knowledge and practice of effective crisis diversion skills.
  • · Can demonstrate a working knowledge and skills in the following, at a minimum: developmental life span; State laws for reporting abuse and neglect; safety planning; and service planning.
  • · Understands case management / care coordination and the referral, linkage, and advocacy process.
  • · Is committed to utilizing evidenced based practices and interventions.
  • Supervisory Relationships
  • Works under the direct supervision of the Adult Services Supervisor.
