Responsibilities
- Provide leadership and direction to a team of case managers and support staff.
- Conduct regular supervision meetings, offer guidance, and support professional development.
- Ensure adherence to professional standards, ethical guidelines, and organizational policies.
- Develop and implement case management programs and initiatives to enhance service delivery.
- Establish goals, objectives, and outcome measures for the department.
- Design and implement standardized processes and protocols for case management.
- Monitor and evaluate the quality of case management services provided.
- Ensure compliance with relevant regulations, licensing requirements, and accreditation standards.
- Develop and implement quality improvement initiatives based on identified needs.
- Collaborate with other departments and stakeholders to ensure integrated and coordinated care.
- Foster effective communication and collaboration among the case management team and other departments.
- Participate in interdisciplinary meetings and committees to promote effective care coordination.
- Manage the allocation of resources, including staffing, budget, and equipment.
- Develop and monitor departmental budget in collaboration with the finance department.
- Identify and advocate for additional resources or program enhancements as needed.
- Develop and implement performance evaluation systems for case management staff.
- Collect and analyze data to track outcomes, identify trends, and generate reports.
- Prepare and present reports to senior management and relevant committees.
- Master's degree in Social Work (MSW) from an accredited program.
- Licensed Clinical Social Worker (LCSW) in the respective jurisdiction.
- Previous experience in case management, preferably in a supervisory or managerial role.
- Strong knowledge of case management principles, best practices, and relevant regulations.
- Excellent leadership, communication, and interpersonal skills.
- Ability to collaborate effectively with multidisciplinary teams.
- Strong analytical and problem-solving abilities.
- Familiarity with electronic health record systems and case management software.
- Knowledge of healthcare reimbursement systems and utilization management is desirable.
