SummaryThe LCSW Manager/Director of Case Management is a leadership position responsible for overseeing and managing the case management department within a healthcare organization or social services agency. The primary objective of this role is to ensure the efficient and effective delivery of case management services while maintaining high-quality standards of care. The LCSW Manager/Director of Case Management will report to the senior management team and collaborate with other departments to ensure integrated and coordinated care for clients.
Responsibilities
  • Provide leadership and direction to a team of case managers and support staff.
  • Conduct regular supervision meetings, offer guidance, and support professional development.
  • Ensure adherence to professional standards, ethical guidelines, and organizational policies.
  • Develop and implement case management programs and initiatives to enhance service delivery.
  • Establish goals, objectives, and outcome measures for the department.
  • Design and implement standardized processes and protocols for case management.
  • Monitor and evaluate the quality of case management services provided.
  • Ensure compliance with relevant regulations, licensing requirements, and accreditation standards.
  • Develop and implement quality improvement initiatives based on identified needs.
  • Collaborate with other departments and stakeholders to ensure integrated and coordinated care.
  • Foster effective communication and collaboration among the case management team and other departments.
  • Participate in interdisciplinary meetings and committees to promote effective care coordination.
  • Manage the allocation of resources, including staffing, budget, and equipment.
  • Develop and monitor departmental budget in collaboration with the finance department.
  • Identify and advocate for additional resources or program enhancements as needed.
  • Develop and implement performance evaluation systems for case management staff.
  • Collect and analyze data to track outcomes, identify trends, and generate reports.
  • Prepare and present reports to senior management and relevant committees.
Qualifications
  • Master's degree in Social Work (MSW) from an accredited program.
  • Licensed Clinical Social Worker (LCSW) in the respective jurisdiction.
  • Previous experience in case management, preferably in a supervisory or managerial role.
  • Strong knowledge of case management principles, best practices, and relevant regulations.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to collaborate effectively with multidisciplinary teams.
  • Strong analytical and problem-solving abilities.
  • Familiarity with electronic health record systems and case management software.
  • Knowledge of healthcare reimbursement systems and utilization management is desirable.
