  • JOB DESCRIPTION: Job Title: Certified Family Partner
  • Youth and Family. Location: Office in Texas City, TX. Serves Brazoria and Galveston Counties. FLSA Status: Non-Exempt. Working Status: Full Time with Benefits. Summary: Implements family support services to LAR (legally authorized representative)/(Parent) of a child or adolescent with an emotional or mental health issue. Provides support and parent mentoring, information, referral, follow-up, and technical assistance to families, teaches self-advocacy skills, and general consultation to staff on meetings with parents of children and youth.

  • YBHS will be responsible for the following essential Job Duties: Ensure all documentation is complete, accurate, and clear as required by GCC Consumer Records Procedure.
Ensure all billing and progress notes are completed in the Electronic Health Record within 48 hours of service delivery. Scan and upload all external records received to the Electronic Health Record at a minimum of weekly to ensure staff access to the most up-to-date patient information. Consult with NAMI and clinical staff as appropriate on the delivery of Certified Family Partner Services. Obtain ongoing training/education in the area of TRR for children and adolescents. Assist families in locating and utilizing available services. Identifies and documents barriers to utilizing services for all referrals. Maintain CEUs required for CFP certification. Meets or exceeds the established performance requirement for the entire caseload each month. Meets all productivity measures determined by leadership staff. Meet with families assigned through the Wraparound process at least weekly for LOC 4 and YES Waiver youth. Meet with LAR of youth in LOC 2, 3, 4, and YC to ensure meeting family partner measures set forth by state contract. Assist with the facilitation of families' engagement in the Wraparound process by providing support around the development of the child and family team (CFTM) and through participation in monthly child and family team meetings. Provide peer support, advocacy, and/or mentoring to other families as identified who are not receiving Wraparound services. Facilitate support groups for families on a monthly basis to ensure the agency's compliance with state contracts. Meet with LAR and youth to provide Medication Training & Support to families as identified on the Recovery Plan. Meet with LAR of youth to provide Nurturing Parenting as identified on the Recovery Plan. Implement Fish philosophy in approach to assigned work duties. Abide by principles of Fish's philosophy when collaborating with co-workers and when interacting with youth and their families. Develop and maintain an up-to-date, usable directory of appropriate community referrals. Support and encourage families to educate themselves by attending parent education classes and providing them with material pertaining to mental health issues, educational issues, and other pertinent issues. Work to ensure service provision remains within the boundaries of the Certified Family Partner role. Accept clinical direction from QMHP staff when appropriate to ensure working boundaries are maintained. Actively participate in team meetings and other clinical staffing as determined by the supervisor. Remain attentive with professional mannerisms throughout the duration of meetings. Display professionalism and team membership when representing the program throughout daily work. Check and respond to e-mail communications in a timely manner. Maintain communication with immediate supervisor regarding flextime. Strict adherence to the Certified Family Partner Code of Ethics and Code of Conduct. Maintain report of monthly purchases made with office VISA cards and YAGA funds as requested/needed. Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop, and continue our community partnerships as they relate to the position. Treat all individuals with the utmost courtesy, decency, and respect. Provide responsive, and efficient services. Interact with staff, consumers, local entities, and advocacy groups to provide a service that meets local needs. Provide the highest level of Customer Service for both internal and external customers. Required to report personnel leave; flex time and emergency leave to assigned supervisor. Strict adherence to the professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct as well as the Corporate Compliance Plan. Consistent adherence to the Center's Dress Code. Consistent adherence to Center safety guidelines and expectations. May be required to travel within Brazoria and Galveston Counties. Out-of-region travel may be required to attend training. Must maintain a clean driving record and driver's license in accordance with our Driver Policy. Requires current Texas Driver's License, reliable transportation, and must maintain current auto liability insurance. Supervision: Receives direct supervision from Program Manager-YBHS. Required Education: Requires a high school diploma or equivalent. Required Licensure/Certifications: Current Family Partner Certification (CFP) OR Family Partner Certification (CFP) obtained within the first 6 months. May require varied work hours depending on the needs of clients. Out-of-region travel may be required to attend training. May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance. Special Projects: Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position. Ability to push/pull, crawl/squat as it relates to PMAB requirements; may be required to perform PMAB restraints as it relates to program requirements. Ability to detect and write effectively. Ability to hear (with or without aid). Ability to have visual acuity to operate a motor vehicle and observe objects within the work setting. Mental: Ability to accept constructive criticism. Ability to maintain regular attendance. Ability to understand, communicate, and comprehend accurate information with others in English. Ability to reason and analyze effectively. Ability to solve practical problems as it relates to the position. Required to pass and maintain all center-mandated training. Work Site/Environmental Conditions: Ability to work alone. Ability to work closely with others. Ability to work closely with clients and/or family members with poor hygiene. Ability to work long and/or irregular hours.
