JobSummary:

Primary function is to provide healthcare and relatedservices to the patient in their place of residence; assist in providing a safeand clean environment, work cooperatively with patient and family and shareobservations and problems with the supervisor. Works under the direction andsupervision of a registered nurse.

JobQualifications:

Education: HighSchool Diploma preferred. Has successfully completed one of the following: A)Training program/competency eval as specified in §418.76 (b) (c) of theMedicare regulations; B) Competency eval program that meets §418.76 (c); C)Nurse Aide training/competency eval program approved by the state and listed ingood standing on the state nurse aide registry; D) State Licensure program thatmeets requirements of §418.76 (b)(c).

Licensure: Musthave current Driver’s License

Experience: Atleast one year preferred, as a hospice aide or nursing assistant in a hospital,nursing home, home health agency, hospice, or long-term care facility orcompletion of Certified Nursing assistant or Certified Home Health AideProgram.

Skills: Mustbe able to read and write in English and follow written and verbal instructionsin English effectively. Demonstrates interest in the welfare of the ill andelderly. Must pass Aide CompetencyEvaluation Skills Checklist that meets §418.76(c) and written skills test.

Transportation: Reliable transportation. Valid andcurrent auto liability insurance.

Criminal History: Must agree to and pass a criminalhistory check.
