Position Overview: As a Direct Care Associate at Creative Options, you will be responsible for providing direct care in a center-based and community-based setting. You will assist the Program Supervisor in providing direct service treatment and case coordination for adults with Developmental Disabilities. Your role will involve implementing individualized support plans based on each person's Person-Centered Plan, assisting adults in developing essential life skills, and promoting independence. You will engage in community-based activities and outings, foster meaningful relationships within the community, provide guidance in problem-solving and conflict resolution, and assist in managing social and behavioral needs to promote a positive and inclusive environment.

Community Served: Adults aged 18 and over with a diagnosed developmental disability.

- Assist in managing social and behavioral needs, promoting a positive and inclusive environment.
- One year of direct care experience working with adults with Intellectual and Developmental Disabilities or mental illness (Certified Nursing Assistant,CNA a plus).
- Good knowledge of developmental disabilities, autism, and other developmental disabilities.
- Demonstrated skill in working with adults with disabilities.
- Excellent oral and written communication skills.
- Ability to establish and maintain effective working relationships with clients, families, employers, and other service provider staff.
