Social Worker / Client Acquisition Specialist and Case Manager

Position Overview:

The Social Worker / Client Acquisition Specialist and Case Manager is responsible for establishing relationships with potential clients and converting leads into loyal clients. The candidate will also manage and monitor the service provisions to ensure that our clients receive the highest quality care.

Key Responsibilities:

Client Acquisition and Relations:Establish initial contact with potential clients and/or their families to assess needs.
Convert potential clients (leads) into active clients by presenting and tailoring our services to their specific needs.
Build and maintain relationships with community organizations, local healthcare providers, and other relevant entities to enhance our referral network.

Case Management:Conduct comprehensive assessments of client needs and develop personalized care plans.
Continuously monitor, evaluate, and adjust care plans as necessary.
Liaise with caregivers to ensure client satisfaction and address any concerns or changes in client condition.
Ensure that services are delivered according to regulatory standards and company policies.

Documentation and Reporting:Maintain detailed records of client interactions, care plans, and service provision.
Prepare and present reports detailing client progress and any challenges encountered.
Ensure all client documentation complies with state regulations and agency policies.

Team Collaboration:Collaborate with internal teams, including caregivers, nursing staff, and administrative personnel, to ensure holistic care.
Participate in regular team meetings and offer insights into client needs and care improvement.

Qualifications:

• Bachelor's or Master’s degree in Social Work, preferably licensed in Massachusetts.
• Minimum of 2 years of experience in a home care setting, healthcare environment, or related field.
• Strong interpersonal and communication skills.
• Proven track record in sales, client relations, or business development is a plus.
• Comprehensive understanding of the Massachusetts regulations pertaining to non-medical home care.
• Proficient in electronic health records and Microsoft Office Suite.
• Ability to multitask, prioritize, and handle a high-pressure environment.
