Department: Harbor Care Health and Wellness Center

Reports to: Nurse Manager

Pay Status: Full Time/Hourly

Work Schedule:Generally, Monday through Friday; 8:00am – 5:00pm

Location: Nashua

The Job: The Registered Nurse (RN) is a professional employee, trained to provide skilled personal care and health related services. The RN functions under the direction, instruction and supervision of the Nurse Manager/Program Manager designee.

Duties & Responsibilities

  • Provides anticipatory guidance, screening and health education to patients to promote and maintain
  • Initiates treatment for patients with commonly occurring health problems which lie within the nurse specialist’s scope of practice by using a systematic approach to history and physical appraisal of patients and/or families through clinical judgment of evaluated data. Prescribes, coordinates, delegates and evaluates
  • Responsible for tracking and outreach of patients in the Hepatitis C program.
  • Monitors patients with stabilized long term or chronic illness and aids and supports individuals and their families
  • Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
  • Instructs individual and/or families or groups about health or defined health problems.
  • Participates in Quality Assurance activities.
  • Participates with providers to ensure timely and effective care for patients.
  • Participates in education of graduate nursing students, as assigned.
  • Promotes community relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality and dignity.
  • Maintain a neat, clean and professional appearance and work space.
  • Appropriate footwear for all seasons.
  • Conduct oneself in a manner that promotes safety.
  • Complete all necessary paperwork related to the position and/or yourself in a timely manner.
  • Other duties as required by Program Manager and/or designee.

Job Requirements

Basic Qualifications:

  • Associates Degree in nursing from an accredited school of nursing.
  • Two (2) years of Medical/Surgical experience as a Registered Nurse.
  • Current registration and licensure in the State of New Hampshire.
  • Current CPR certification,

Preferred Qualifications:

  • Bachelor’s degree is preferred but not required.
  • Resourcefulness, flexibility and integrity.
  • Experience working in a Federally Qualified Healthcare Center organization is a plus.
  • Experience in a community healthcare setting.
  • Knowledge of Microsoft Office and Centricity EMR software preferred.

Competencies

  • Good oral, written and reading communication skills. Have the ability to understand verbal and written instructions. Read and write documentation in patient record.
  • Be aware of and adhere to HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency.
  • Resourcefulness, flexibility and integrity.
  • Respectful of people and their cultural, religious and ethnic differences.
  • Ability to function in a team-oriented environment.
  • Attention to detail.
  • Proficient in taking vital signs and basic RN skills.
  • Ability to make meaningful observations and document these findings.
  • Ability to organize and prioritize work.
