In this Licensed Practical Nurse role, you will be providing in home high quality skilled nursing care to patients in the

Boca, Boynton, Delray area , FL area. Under the direction of the Plan of Treatment established with a physician and with input from patient/caregiver, you will assist in ensuring a better way of living and healthy recovery. As an LPN, you would report problems/potential problems to RN/Case Manager.

*Must be willing to cover patients in the Boca, Boynton, Delray area.

Role Responsibilities:

• Participation in the nursing process by providing input to the client's plan of care
• Implementing the care plan as written
• Evaluating effectiveness of care plan
• Record a patient’s medical history accurately
• Provide emotional and psychological support when needed
• Communicate with patients’ family or friends to provide advice, comfort and release instructions
• Other responsibilities will be assigned

Haven Qualification Standards: 

• Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA
• 1- year of skilled Home Health experience required
• High tech skills, IV certification a plus! 
• Proven experience as LPN or similar role
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
• Understanding of confidentiality obligations and nursing best practices
• Excellent communication and interpersonal skills
• Compassionate and able to handle stress
