Position Summary: We are seeking PRN dedicated and compassionate Home Hospice RN Case Manager to join our team in the Tucson and surrounding areas. The ideal candidate will have a strong background in hospice care and be committed to providing high-quality, patient-centered care.

Key Responsibilities:

• Provide direct patient care in the home setting, managing symptoms and addressing patient and family needs.
• Develop and implement individualized care plans based on patient assessments.
• Coordinate with interdisciplinary team members, including physicians, social workers, and chaplains, to ensure comprehensive care.
• Educate patients and families on disease progression, symptom management, and end-of-life care.
• Monitor and document patient progress, making adjustments to care plans as necessary.
• Participate in on-call rotation to provide support to patients and families as needed.

Qualifications:

• Active RN license in the state of Arizona.
• Minimum of 1 year of hospice nursing experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and empathetic approach to patient care.
• Valid driver's license and reliable transportation.
