POSITION GOAL

This position is responsible for building and maintaining a partnership with veterans and families that experience homelessness. The position works with at risk and homeless veteran households who are enrolled in the Shallow Subsidy and standard Rapid Rehousing and Homeless Prevention services in the Support Services for Veteran Families program. The case manager works with each household to develop housing stabilization plans to obtain and maintain permanent housing and working with veterans to increase income and access to other community supports. The case manager may maintain a case load of veterans in a 12 county service area. This position will work with other team members in the Support Services for Veterans Families (SSVF) program and HACAP’s other homeless services programs. This position would be based in Cedar Rapids but weekly driving will be expected to other communities across a 12 county service area. The role will be a hybrid role of working with veterans enrolled in the Shallow Subsidy initiative and veterans who are also enrolled in standard SSVF services.

SCHEDULE AND WAGE

Monday - Friday 8:00 am- 5:00 pm (some non-traditional hours may be needed); $24.86 hourly. 

*This position is eligible for a $1200 retention incentive after successfully completing 180 days of employment.

ESSENTIAL DUTIES

  1. Responsibilities and tasks include a wide range of community networking activities to assist veteran households who are enrolled in the Shallow Subsidy service and who have been placed in housing.
  2. Connect and assist veterans who risk of losing their housing and/or who are homeless and in need of securing traditional, permanent housing with safe and permanent housing options.
  3. Maintains precise and accurate documentation of case management services, including housing stabilization plans, community referrals, community networking activities in the correct database.
  4. Adhere to the policies and procedures outlined in the Housing Program Manuals (SSVF and HACAP policies and procedures)
  5. Work with supervisor and compliance staff on turning in all required case management and housing stabilization documentation.
  6. Assists veterans and other homeless households with completing applications for benefits, employment, social security and other mainstream resources.
  7. Assists veterans in enrolling in other VA services they may be eligible for.
  8. Advocates for needed services and assist landlords in helping clients meet the obligations of tenancy.
  9. Establishes and maintains collaborative working relationship with community resources, including the programs at the VA Medical Center; attends and participates in appropriate coalition and other community resource meetings.
  10. Provides case management services, including employment referrals and counseling, at least monthly to an assigned caseload of veterans.
  11. Conduct meetings at a time convenient for veterans. This will include both daytime and evening weekday hours.
  12. Attend case conferencing and all other meetings required by management
  13. Other duties as assigned by management staff needed by the agency for program operations.

QUALIFICATIONS

TARGETED EDUCATION and/or EXPERIENCE

1. Bachelor’s degree in Human Services or related field and two years’ experience;

OR

2. 7 years work experience in homeless services field

3. Working knowledge of community resources as they relate to homelessness, veterans, and affordable housing.

4. Experience in assisting individuals with employment skills and building income.

5. Knowledge of veteran homeless services and VA services

6. Service in the military preferred or employed in veteran services preferred

LICENSES AND CERTIFICATIONS

  • Valid Iowa driver's license and proof of insurance
  • SOAR Training (preferred not required)

 
PHYSICAL REQUIREMENTS

While performing duties of this job, the employee is regularly required to sit and work at a computer for long periods of time in an office environment and must be able to lift/move weight up to 25lbs. Employee is also required to accept incoming calls and must be able to hear and speak. Must have visual acuity to read and draft reports, memos, letters, etc. Specific vision abilities required by this job include vision adequate for the incumbent to perform the responsibilities and functions of the job efficiently. Must have the manual dexterity to manually operate and use a computer. Incumbent must be able to drive in all weather and road conditions. The noise level is quiet to moderate; non-­-smoking environment.


WORK ENVIRONMENT

The employee will work in an office and public, service-­-delivery environment. Hectic situations can occur in the public area of the facility characteristic to working with the homeless community and the related concerns and situations related to homeless conditions. Potential exists for volatile and unpredictable situations to occur. May experience exposure to strong odors such as soiled clothes and poor hygiene.
