SUMMARY Hayes Gibson Property Services (HGPS) is looking for an experienced Service Coordinator in which is responsible for the coordination of the Social Service functions of the residents at Bowling Green Towers. The Service Coordinator plays a critical role to support assisted housing as a platform for financial security, physical security, social connections, and the delivery of long-term community based supportive services. This includes oversight of the daily social services operation, community collaborations, development of future programs, statistical and program reporting, monitoring of program funding and reporting requirements. Requirements QUALIFICATIONS
Experience in Social Services/Case Management
Proven work experience in similar roles
Solid communication skills, both written and verbal, and works well with others
Professional appearance and demeanor
Excellent phone etiquette and ability to maintain a professional attitude
Good with basic computer operations
Ability to be resourceful and proactive in dealing with issues that may arise
Ability to organize, multi-task, prioritize and work under pressure
Valid Driver's License Required

Assist residents to access services available in the community, and designs programs and services to meet the needs and desires of the residents
Foster an environment in which elderly individuals and individuals with disabilities can live independently and remain in the community
Provide oversight and coordination of social services for respective residents
Responsible for social care coordination services that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being
Maintain knowledge of current and future social service and community trends and assist of social service programs
Monitor social services spending, including grant funding
Assist with presentations and community functions
Facilitate respective resident's case management and coordination of services including but not limited to dietician services, financial services, meal planning, transportation services, etc.

Greet residents and establish rapport
Present and provide follow through as a helping professional worthy of trust
Demonstrate understanding of how to view residents as partners in the assistance process and as experts in their own lives
Work to strengthen resident sufficiency and external supports; empower residents to become more independent of emergency services
Through relationship building, develop and promote local and community services to residents and referral sources
Based on designated care plan of each resident, provide assistance through internal resources and make referrals to outside resources as available and appropriate
Listen for and assess for other needs including food, clothing, and emotional support
